Dear RIS/RIST Faculty and Staff,
As the current Health Insurance card is going to be expired
on June 30, 2010 thus, we are in a process of issuing a
new card for health insurance year of 2010-2011.
The survey for you to choose your health insurance option
will be given out soon. However, for those who prefer to
change their picture on the card or those who last year
did not have a picture on their card, you can send your
picture in JPEG file ONLY to adviser-health@rism.ac.th Please
make sure that the file is not too small otherwise the picture
would not be clear when print on the card.
The deadline for picture is on April 15, 2010 after the
deadline we will not accept any picture and your card will
be printed with your previous picture or with no picture
(in case you never have picture on the insurance card )
